Kcnq1, the mouse ortholog of human KCNQ1, encodes Kv7.1, a voltage-gated potassium channel whose gating is transformed by accessory KCNE subunits: with KCNE1 it produces the slowly activating cardiac current IKs that provides repolarization reserve during sympathetic drive, while with KCNE3 it becomes a constitutively open conductance supporting chloride secretion in intestinal and airway epithelium. Activity requires PIP2 and calmodulin bound to the carboxy-terminal helices, and PKA phosphorylation through yotiao (AKAP9) mediates beta-adrenergic augmentation. Loss-of-function mutations cause long QT syndrome type 1 and, when biallelic, Jervell and Lange-Nielsen syndrome with deafness from failed endolymph secretion in the stria vascularis, whereas gain-of-function alleles cause short QT and atrial fibrillation. Mouse nulls are viable but deaf with vestibular dysfunction and gastric hyperplasia, so tissue-restricted conditional deletion separates cardiac, cochlear, and epithelial endpoints, and humanized knockins are preferred when the readout is IKs pharmacology, since murine repolarization depends on different currents.
