Despite its name, Ndufa4, the mouse ortholog of human NDUFA4, encodes a subunit reassigned from complex I to complex IV, where it binds the cytochrome c oxidase monomer and prevents dimerization, acting as a bona fide fourteenth COX subunit that modulates catalytic efficiency. Human biallelic NDUFA4 variants cause Leigh syndrome with isolated complex IV deficiency, consistent with that reassignment and providing a rare case in which the gene name actively misdirects experimental design. Allele planning should therefore be built around complex IV endpoints, with a conditional null using neuronal or cardiac Cre to test tissue thresholds for COX activity and patient variant knockins to model the hypomorphic human state. Importantly, the paralog Ndufa4l2 is induced by HIF1A in hypoxic and tumor tissue and substitutes at the same site with altered kinetics, so compound or hypoxia-controlled designs are necessary to avoid masked phenotypes.
