Fgf14, the mouse ortholog of human FGF14, encodes intracellular FGF homologous factor 4, a nonsecreted beta-trefoil protein that binds the C-terminal tails of NaV1.1, NaV1.2, and NaV1.6 and physically associates with CaV2.1 and scaffolding partners at the axon initial segment of cerebellar granule and Purkinje neurons, where it sets sodium channel availability, resurgent current, and the fidelity of high-frequency firing. Human heterozygous missense and truncating variants cause spinocerebellar ataxia 27A, and a dominantly inherited intronic GAA repeat expansion causes the common late-onset ataxia SCA27B, giving two mechanistically distinct disease classes, loss of protein function versus repeat-driven transcriptional silencing. Allele design should therefore fork explicitly: patient missense knockins for channel-interaction deficits, humanized repeat-containing knockins for expansion biology and antisense or small-molecule correction, and floxed conditionals for cell-type attribution using Pcp2-Cre or Atoh1-Cre. Nulls are viable with ataxia and paroxysmal dyskinesia, and paralogs Fgf12 and Fgf13 only partially overlap in cerebellum.
